How to register with us ?

Using an online form

We are using a new online service that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

You can check if you're eligible and register using an online form.

The registration process takes around 15 minutes to complete.

You may print off the registration form, complete it, and bring it with you on your first visit to the practice.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

We can also offer an appointment with one of our Healthcare Assistants who will take some basic tests, height, weight, blood pressure and urine testing. Please ask at reception if you would like to book an appointment.

If you feel you may run out of medications before your registration with us is actioned, please request these from your current surgery.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP. Your named doctor will be allocated to you by the practice.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.

GP registration transfer cards

Accessing healthcare can be a challenge for people seeking asylum who live in Home Office accommodation. Not only do language and communication barriers create access issues, but the temporary nature of the accommodation can also disrupt care when individuals are relocated at short notice.

To help tackle this, NHS England has released GP registration transfer cards available in 31 different languages. The aim is to improve continuity of care for vulnerable migrants and reduce the risk of duplicate records.

We recommend using these cards before any relocation, ensuring that a suitable interpreter is available for the discussion, if needed.
