If you’ve been off sick for more than 7 days

You can request a sick note in one of the following ways:

If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note. Please complete this form. We will contact you to let you know when you can collect your Sick/Fit Note or we may contact you to arrange an appointment.

  • By phone or in person: You can contact reception to request a sick note or discuss the process of booking an appointment.

Please be aware that a clinician may need to review you before issuing or extending a sick note. This may be done by reviewing your request form, or you may be asked to arrange an appointment if further assessment is required.

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.
